Six weeks of personalized health coaching includes 1:1 weekly sessions, ideal for clients seeking guidance from a healthcare provider coaching perspective. This plan is designed to help individuals reach short-term goals, gain clarity and introspect to improve their quality of life, manage stress, and enhance overall wellness.

Experience eight weeks of personalized health coaching through 1:1 weekly sessions. This plan is recommended for clients who may not have multiple complexities but need more time and support to address personal, health, or professional needs to reach and sustain targeted goals.

Includes twelve weeks of personalized coaching through 1:1 weekly sessions, complemented by email and call support between meetings. Recommended for those navigating new or multiple diagnoses, chronic issues or life complexities such has job loss, divorce, stress, poor eating habits, etc. that require more time to adapt to and achieve sustainable change.

Clients receive five, once weekly nurse coaching sessions free of charge. This includes one initial intake session and four follow-up sessions.

This session provides for collection of pertinent data about your personal needs, professional goals, obstacles to success, medical history, and other pertinent details to assess your needs and how we may best advocate for you.

Outpatient Advocacy Services provides in-person support to individuals in the outpatient setting. We will collaborate with your provider to assist in understanding your plans of care, diagnoses, treatment options, medications, and expectations to increase compliance and optimize your outcomes. Also includes one week of follow up and call support. Availability and pricing vary depending on location and specialty.

Professional support and guidance for hospitalized patients and their families. One in-person visit to the facility and/or the bedside to meet collectively with the patient, family, and/or providers to help facilitate communication with providers, navigate the healthcare system, explore treatment options, facilitate mutual understanding with decision making, plans of care, and discharge education to optimize post-hospitalization outcomes and follow up.

Retention fee is $250. The hourly bill rate varies based on the complexity of the review. This can help establish a timeline of events, identify potential patterns, oversights, and discrepancies in care and documentation, and assess continuity and congruency of care. The findings will be gathered into a written report to provide clarity, transparency, understanding, and closure.
